Source status
Every adapter shows its last successful run and its triage kill rate, the share of harvested documents discarded because they contain no ask. When a kill rate moves sharply we treat the source as broken and say so here, publicly, until it is fixed.

Duplicates across sources are good: the same document arriving via two adapters validates both. Documents are hashed on arrival; the archive keeps the copy the claims point at.
